Which form this covers: The card covers the IV induction doses for ulcerative colitis and Crohn's disease, with a separate yearly amount for the infusion visit and labs. SC maintenance doses and psoriasis use are mostly self-injected through the pharmacy.

Who qualifiesIncome: at or below 400% of the federal poverty level (2026: $62,600 for 1 person, $84,600 for 2, all states except AK/HI). Insurance: Uninsured, OR commercial/employer OR government (Medicare etc.) coverage; must be inadequate. Medicare Part D patients at or below 150% of the federal poverty level must show LIS ineligibility. Outpatient; US, PR, USVI.
