Which form this covers: Tezspire is mostly self-injected and paid through the pharmacy benefit. This page applies when the clinic bills it under the medical benefit. The program's terms cover claims for both routes.

Who qualifiesCommercial or private insurance that covers TEZSPIRE, including exchange plans, regardless of financial need; no income requirement. Not for patients whose drug or in-office administration is paid in whole or part by Medicare, Medicaid, TRICARE, DoD, VA or other government programs.

What it pays forThe Tezspire card pays for the drug and the administration (infusion or injection) fee.
