Who qualifiesCommercially insured, 18+, US/territories; commercial coverage must not cover full cost; not cash-pay. Excluded: Medicare, Medicaid, Medigap, CHAMPVA, TriCare, VA, DoD, any state/patient/pharma assistance program.
What it pays forThe Opdivo card pays for the drug only, not the infusion or injection fee.
Clinic-billed (medical benefit) claimsYes. Submitted by: the provider or the patient. Claim deadline: 180 days from date the claim was processed; retroactive to out-of-pocket expenses within 180 days before enrollment.
