Who qualifiesCommercially (privately) insured with an approved indication; not valid with Medicare (incl. Part D), Medicaid, Medigap, TRICARE, VA, DoD, or other federal/state programs.
Clinic-billed (medical benefit) claimsYes. Send the claim within 180 days of the date of service.
How to submitSubmit the EOB and a 1500 claim form. Sandoz One Source page header: Co-Pay Claims Fax 1-833-227-9947; Co-Pay Claims Mail: 2250 Perimeter Park Drive, Suite 300, Morrisville, NC 27560; HCPs can Upload Documents (Submit co-pay claims). Program fax 1-833-671-1084 for enrollment.
Plans that count only part of the cardIt is a violation of the terms and conditions of this program to use it to enroll patients for the purposes of a copay accumulator or maximizer program. Also: Restrictions, including monthly and/or annual maximums may apply. Patient is responsible for any costs once program limit is reached. No dollar figure published.
