Who qualifiesIncome: Uninsured patients with incomes under $150,000; insured patients without coverage for the drug with incomes under $150,000; insured patients WITH coverage if their plan out-of-pocket maximum exceeds 7.5% of household income, who have pursued other financial help, with household size/income within guidelines. Add $25,000 per extra person in households larger than 4. Insurance: Uninsured OR insured (with or without coverage for the drug).
