Who qualifiesCommercial insurance only; valid Rx for AMVUTTRA or ONPATTRO; age 18+; US/territories. Not valid under Medicare, Medicaid, TRICARE, VA, DoD, IHS, or any federal/state program; not where patient is not using insurance, insurance pays the entire cost, or product is not covered.
What it pays forThe Amvuttra card pays for the drug and the administration (infusion or injection) fee.
Clinic-billed (medical benefit) claimsYes. Submitted by: the provider or the patient. Claim deadline: Postmarked within 180 days of date of administration.
How to submitOne form per patient per date of service, plus copy of the EOB or billing statement for the date of administration; fax 800-984-8816 or email AlnylamCopay@UBC.com. Payment by EFT to the bank account on file or check delivered by email 3-5 business days after processing. Program Support 844-985-4498.
Plans that count only part of the cardTerms: program value "is intended to be credited towards patient out-of-pocket obligations and maximums, including.. any programs offered by a third party payer or pharmacy benefit manager.. that adjusts patient cost-sharing obligations, through arrangements that may be referred to as accumulator or maximizer programs." (i.e., Alnylam states its assistance is meant to count). Program has an annual benefit cap.
