How to calculate Xolair billing units

Xolair J2357 is billed in units of 5 mg: ordered dose ÷ 5 mg = billing units. Unlike a flat-dose drug, the ordered dose itself is not fixed: chronic spontaneous urticaria is 150 or 300 mg, and asthma, nasal polyps and food allergy are read off the label's serum IgE × body-weight table, from 75 mg up to 600 mg. Whichever dose is ordered, the unit math is the same division by 5: a 300 mg dose is 300 ÷ 5 = 60 billing units.

Xolair ships as single-dose prefilled syringes in three sizes (75 mg, 150 mg and 300 mg), and every dose is built from whole syringes with the least waste, never a partial draw: 300 mg is one 300 mg syringe, 225 mg is a 150 mg plus a 75 mg syringe, and so on. Because nothing is drawn and left over, JZ (no drug discarded) is the modifier on virtually every Xolair claim, and there is no JW math to do.

The dose is set once. Serum total IgE and body weight, measured before the first injection, fix the dose and interval from the table for the whole course; the label does not call for re-dosing on an IgE level drawn during treatment. Chronic spontaneous urticaria is the exception: its 150 or 300 mg dose was never dependent on IgE or weight to begin with.

Medicare pays $9.1374 per mg (ASP+6%, $45.687 per 5 mg unit), effective for claims in Q4 2026, pricing the 60-unit, 300 mg dose above at $2,741.22.

Every dose in the label's IgE × weight table is built from whole 75, 150 and 300 mg syringes with the least waste; the engine never draws a partial syringe. Two-syringe doses (225, 375 and 600 mg) are two separate injections and two 96372 lines, the second reported with modifier 59 or XS on most payers' edits. The MUE for J2357 is 120 units per day, exactly the 600 mg ceiling in this table; 96372's own MUE is 4. Chronic spontaneous urticaria uses only the 150 mg and 300 mg rows; asthma, nasal polyps and food allergy can land on any row, depending on the patient's pre-treatment IgE and weight.

96372 covers a therapeutic, prophylactic, or diagnostic injection given subcutaneously or intramuscularly. Xolair's subcutaneous injection is billed with 96372 alone; there is no infusion time to report, so the infusion-administration codes (96413 initial hour, 96415 each additional hour) used by IV biologics do not apply. Xolair is also not a chemotherapeutic agent, so the chemo-injection code 96401 does not apply either. A dose built from two syringes (225, 375 or 600 mg) is two separate injections and two 96372 lines, not one line at a higher unit count. The visit itself typically runs about 30 minutes, including an observation period after the injection.
