Injectafer’s card can’t be used with Medicare. These funds may help. They open and close with donations, so call before telling a Injectafer patient no.

Commercial insurance — The Injectafer Savings Program is American Regent's copay card: patients pay $50 per dose, and the program pays up to $500 per dose, up to $2,000 per calendar year, capped at four doses per calendar year. It excludes Medicare, Medicaid, TRICARE, any other federal health care program, and Medicare-eligible retirees on an employer plan. It covers the drug only — the 96374 administration fee and the office visit are billed separately. View current program ↑
Uninsured or a coverage gap — Injectafer Patient Assistance Program ships Injectafer free directly to the provider for patients who are uninsured or have a coverage gap, at or below 500% of the federal poverty level, with no cap on the number of doses. The shipment is the drug alone — the office visit and the 96374 push fee still go on the patient's bill.
Medicare — Manufacturer copay savings cannot be used when government insurance is paying for Injectafer; the federal Anti-Kickback Statute is why, per HHS OIG guidance on copayment coupons, and it is also why the Injectafer Savings Program excludes Medicare-eligible retirees on an employer plan. Injectafer Patient Assistance Program is not a fallback either: its eligibility is limited to patients who are uninsured or have a coverage gap. On a 750 mg visit, a Medicare patient's 20% coinsurance is $175.70 on $878.49 allowed against $57.55 with the card ($50 for the drug plus the $7.55 share of the push) — three times as much.
Why the diagnosis selector doesn't change the result — Neither Injectafer program is gated by diagnosis. Eligibility runs on the patient's insurance status, and on household income for Injectafer Patient Assistance Program. IDA with oral iron failure (D50.9), non-dialysis CKD (D63.1), and heart failure (E61.1) all qualify the same way for whichever program matches the patient's insurance.
Yes. The Injectafer Savings Program is American Regent's commercial-only copay card: patients pay $50 per dose, and the program covers up to $500 per dose, up to $2,000 per calendar year, capped at four doses per calendar year. It excludes Medicare, Medicaid, TRICARE, any other federal health care program, and Medicare-eligible retirees on an employer plan. It covers the drug only, not the 96374 administration fee or the office visit. Apply on injectafercopay.com ↗ Calculate cost with the program →
