Commercial insurance: The EntyvioConnect Co-Pay Program Program is Takeda's commercial copay card. Eligible patients pay as little as $0 per dose, up to $20,000 a year. An Entyvio claim carries two lines, the drug (J3380 billed at 300 units for the 300 mg dose) and the 96365 infusion administration, and the program applies to the drug line only; the infusion charge is billed and collected separately. The EntyvioConnect Co-Pay Program Program covers the Entyvio drug charge only, not the 96365 infusion administration fee, and it excludes Medicare, Medicaid, TRICARE, VA, and other government insurance outright.

The EntyvioConnect Co-Pay Program Program is commercial-only, and every foundation fund reachable through J3380 requires Medicare, Medicaid, or TRICARE — or, for the vasculitis fund, a diagnosis Entyvio does not treat.
Why the diagnosis selector doesn't change the result: None of the four inflammatory bowel disease funds gates eligibility between Entyvio's two indications; both list ulcerative colitis (K51.90) and Crohn's disease (K50.90) codes, so switching the diagnosis does not change which programs apply.
