How the lines work: A Vabysmo visit produces two charges: a drug line (J2777, billed per 0.1 mg, so the 6 mg per-eye dose is 60 units) and a procedure line for the injection itself (67028 per eye, reported RT or LT, or once with modifier 50 when both eyes are injected at the same visit). Each line carries its own patient share. The Genentech Ophthalmology Co-pay Program’s record naming “drug and administration costs” makes it the one program in this class that names the procedure line, rather than the drug charge alone.
Medicare: Manufacturer copay assistance is not available to patients using Medicare to pay for Vabysmo; the Genentech Ophthalmology Co-pay Program explicitly excludes Medicare, Medicaid and other government plans. What exists instead is a Medigap policy that picks up the Part B coinsurance (see the cost page for the math) and Medicaid dual eligibility where it applies.
