How the lines work — Tepezza is billed per 10 mg under J3241 for thyroid eye disease, coded to H05.24, a category code; payers recognize the more specific H05.241–H05.249 codes by eye, and the funds above match on the J-code and the thyroid eye disease diagnosis. The example on this page is the 20 mg/kg maintenance dose: for an 80 kg patient that is 1,600 mg, billed as 160 units of J3241 from four 500 mg vials, with the 400 mg remainder reported separately as 40 units on a JW line. The only vial size is 500 mg, so most weights leave some drug discarded this way. The infusion itself is a separate line, 96413, run at least 60 minutes if the first two infusions in the course were well tolerated and at least 90 minutes if not. Each line carries its own patient share under commercial coinsurance, and the TEPEZZA Co-pay Program's terms cover both; the example estimate above applies it to the drug line only. Tepezza has no biosimilar and no same-class alternative billed under its own J-code, so this is the only manufacturer program to check.
