A Stelara claim carries two lines, the drug (J3358 billed at 390 units for the 390 mg induction dose, the worked example on these pages) and the 96365 infusion administration, and the program applies to the drug line only; the infusion charge is billed and collected separately. The Janssen CarePath Savings Program covers the Stelara drug charge only, not the 96365 infusion administration fee, and it excludes Medicare, Medicaid, TRICARE, VA, and other government insurance outright.
Medicare, Medicaid, and TRICARE: Manufacturer copay savings cannot apply once government insurance pays for Stelara, per the federal Anti-Kickback Statute guidance that also keeps the Janssen CarePath Savings Program from covering them. The alternative is a foundation fund: HealthWell's AutoImmune – Medicare Access fund (up to $2,800 a year) covers a Medicare patient, and TotalAssist's Ulcerative Colitis fund (up to $3,000 a year) extends to Medicaid and TRICARE patients with that diagnosis — both closed as of September 7, 2026. After the $283 Part B deductible, a Medicare patient's 20% coinsurance on the 390 mg induction dose is $812.85 — $799.42 on the drug line and $13.43 on administration — against $4,064.25 Medicare allows ($3,997.11 drug plus $67.14 administration). Uninsured: The Patient Assistance Foundation is the one program on file for an uninsured Stelara patient, and it has been closed as of June 14, 2026. No other manufacturer program or foundation fund on file accepts an uninsured patient; every foundation fund reachable through J3358 requires Medicare, Medicaid, or TRICARE — or, for the vasculitis and psoriasis funds, a diagnosis Stelara IV does not treat.

The Janssen CarePath Savings Program reduces what a patient owes on an approved, paid J3358 claim; it does not apply to a denied claim. The denial has to be resolved through the payer's appeal or reconsideration process before the program has anything to apply to.
