Prolia and Xgeva share HCPCS J0897 but not their assistance programs. Prolia is the 60 mg osteoporosis product dosed every 6 months; Xgeva is the 120 mg oncology product dosed monthly. Amgen runs a separate copay program for each — the Amgen SupportPlus Co-Pay Program on this page applies to a patient prescribed Prolia for osteoporosis or treatment-induced bone loss only. A patient on Xgeva for bone metastases, multiple myeloma, giant cell tumor of bone, or hypercalcemia of malignancy needs the Xgeva-specific program instead — billing 60 units when the patient actually received Xgeva (or 120 units for a Prolia patient) is a real and common error on the claim itself, before the assistance application is ever reached.

Assistance for the other products in this family

Prolia shares its class with Jubbonti (Q5136, Sandoz), Stoboclo (Q5157, Celltrion), Conexxence (Q5158, Fresenius Kabi), Enoby (Q5167, Hikma), billed under their own codes with their own manufacturer programs. The programs above apply to Prolia only. If the payer requires Jubbonti, Stoboclo, Conexxence or Enoby instead of Prolia, use that maker’s program below.

Each Jubbonti, Stoboclo, Conexxence or Enoby card shows the date we last checked that program. Copay cards also say whether they pay the infusion or injection fee.

Each biosimilar also has a 120 mg oncology product, the Xgeva equivalent (Wyost, Osenvelt, Bomyntra and Xtrenbo). Those are not listed here, since this page covers the 60 mg osteoporosis line only.

How Prolia assistance works

Commercial insurance — Eligible commercially insured patients may qualify for the Amgen SupportPlus Co-Pay Program. Eligible patients may pay as little as $25 per dose, subject to current program terms. View current program ↑

Medicare — Manufacturer copay savings cannot be used when government insurance is paying for Prolia; the federal Anti-Kickback Statute is why, per HHS OIG guidance on copayment coupons. The Amgen Safety Net Foundation is not a fallback either: its covered-product list does not include Prolia. View current options ↑

Uninsured — Amgen’s own free-drug foundation does not cover this product. The Amgen Safety Net Foundation eligibility page names 16 Amgen medicines — Evenity, its other bone drug, among them — and neither Prolia nor Xgeva is on it (checked September 12, 2026). That leaves three real routes: a denosumab biosimilar at cash price, where Hikma publishes Enoby’s list price as $400 a syringe and $800 a year against Medicare’s $1,791.36 allowed amount for 60 mg of reference Prolia; a hospital or FQHC charity-care policy; or a diagnosis-matched foundation fund when one reopens. Check today’s status ↑

Why fund status matters — Manufacturer programs can change between: OPEN Accepting applications now · WAITLIST Not enrolling immediately · CLOSED Not accepting new applications. Every CareCost program card shows the status and the date it was verified.

Why the product name matters — Prolia and Xgeva bill the same HCPCS code (J0897) but are different products with different assistance programs. Confirm which product the patient is actually prescribed before applying.

Frequently asked questions

Is there a Prolia copay card?

Yes. The Amgen SupportPlus Co-Pay Program is Amgen’s current manufacturer savings program. Eligible commercially insured patients may pay as little as $25 per dose. Apply on Prolia.com ↗ Calculate cost with the program →

Can Medicare patients use the Prolia savings program?

No. Patients using Medicare or other government-funded insurance to pay for Prolia are not eligible for the manufacturer’s copay card. The Amgen Safety Net Foundation is not a fallback either — its eligibility page lists 16 Amgen products and Prolia is not one of them. Check current options ↑

Is there help for uninsured patients?

Not from Amgen. The Amgen Safety Net Foundation’s product picker names 16 medicines — including Evenity, Amgen’s other bone drug — and neither Prolia nor Xgeva appears (checked September 12, 2026). What is left for an uninsured patient is a denosumab biosimilar at cash price (Hikma publishes Enoby’s list price as $400 a syringe, $800 a year), hospital or FQHC charity care, or a diagnosis-matched foundation fund if one reopens.
