Commercial insurance: The Ocrevus Copay Card is Genentech's commercial copay card. Eligible patients pay $0 for the drug, up to $20,000 a calendar year, and the program pays administration separately, up to $1,500 in the first calendar year and $1,000 each year after. An Ocrevus claim carries the drug line (J2350, 600 units for the 600 mg maintenance dose from two 300 mg vials) and the administration lines (96413 for the first hour, 96415 twice for the remaining hours of the 3.5-hour infusion); the card can zero out both up to its caps. It excludes Medicaid, Medicare, and other government insurance. View current program ↑
Uninsured: Genentech Access Solutions ships Ocrevus free to eligible uninsured patients at or below 400% of the federal poverty level. Only the drug arrives free; the infusion charge is still billed to the patient separately.
Medicare: Manufacturer copay savings cannot be used when government insurance is paying for Ocrevus; the federal Anti-Kickback Statute is why, per HHS OIG guidance on copayment coupons. The Ocrevus Copay Card excludes Medicare outright, and Genentech Access Solutions covers uninsured patients only. What's available instead is the PAN Foundation Multiple sclerosis (MS) fund, open today, which accepts Medicare and pays up to $8,000 a year toward the drug; HealthWell's Medicare Access fund and Good Days' fund also accept Medicare but are both closed today. On the 600 mg maintenance dose, a Medicare patient's 20% coinsurance is $7,320.69 on the $36,603.45 Medicare allows, once the Part B deductible is met, and no manufacturer program reduces it. Relapsing forms of multiple sclerosis (G35.A), primary progressive MS (G35.B1) and active secondary progressive MS (G35.C1) all qualify the same programs; eligibility runs on insurance status, not on which indication is billed.

The Ocrevus Copay Card and the PAN Foundation funds reduce what a patient owes on an approved, paid claim; neither applies to a denied claim. The denial has to be resolved through the payer's appeal or reconsideration process before any of these programs has anything to apply to.
Ocrevus shares its class with Ocrevus Zunovo (J2351, Genentech), Briumvi (J2329, TG Therapeutics), Tysabri (J2323, Biogen), Lemtrada (J0202, Sanofi), billed under their own codes with their own manufacturer programs.
