Monoferric’s card can’t be used with Medicare. These funds may help. They open and close with donations, so call before telling a Monoferric patient no.

How Monoferric assistance works

Commercial insurance — The Monoferric Patient Solutions Copay Assistance Program Program is Pharmacosmos's copay program: patients pay $0 per dose, and the program pays up to $2,000 per treatment, up to $4,000 per calendar year. It excludes Medicare, Medicaid and government insurance. It covers the drug only — the 96365 infusion fee and the office visit are billed separately. View current program ↑

Uninsured or a coverage gap — Monoferric Patient Solutions Patient Assistance ships Monoferric free directly to the provider for patients who are uninsured or have a coverage gap, at or below 300% of the federal poverty level, with no cap on the number of doses. The shipment is the drug alone — the office visit and the 96365 infusion fee still go on the patient's bill.

Medicare — Manufacturer copay savings cannot be used when government insurance is paying for Monoferric; the federal Anti-Kickback Statute is why, per HHS OIG guidance on copayment coupons, and it is also why the Monoferric Patient Solutions Copay Assistance Program Program excludes Medicare, Medicaid and government insurance outright. The Patient Assistance program is not a fallback either: its eligibility is limited to patients who are uninsured or have a coverage gap, at or below 300% of the federal poverty level. On a 1,000 mg visit, a Medicare patient's 20% coinsurance is $450.97 on $2,254.84 allowed against $13.43 with the card ($0 for the drug plus the $13.43 share of the infusion) — 32 times as much.

Why the diagnosis selector doesn't change the result — Neither Monoferric program is gated by diagnosis. Eligibility runs on the patient's insurance status, and on household income for the Patient Assistance program. IDA with oral iron failure (D50.9) and non-dialysis CKD (D63.1) both qualify the same way for whichever program matches the patient's insurance.

Frequently asked questions

Is there a copay card for Monoferric?

Yes. The Monoferric Patient Solutions Copay Assistance Program Program is Pharmacosmos's commercial-only copay program: patients pay $0 per dose, and the program pays up to $2,000 per treatment, up to $4,000 per calendar year. It excludes Medicare, Medicaid and government insurance. It covers the drug only, not the $67.14 96365 infusion fee or the office visit. Apply on monoferric.com ↗ Calculate cost with the program →

Can Medicare patients use the Monoferric Patient Solutions Copay Assistance Program Program?

No. The Monoferric Patient Solutions Copay Assistance Program Program excludes Medicare, Medicaid and government insurance. On a 1,000 mg visit, a Medicare patient's 20% coinsurance is $450.97 on $2,254.84 allowed against $13.43 with the card ($0 for the drug plus the $13.43 share of the infusion) — 32 times as much. The Patient Assistance free-drug program is not a Medicare fallback either: its eligibility needs the patient to be uninsured or have a coverage gap, at or below 300% of the federal poverty level.

Who qualifies for the Monoferric Patient Solutions Patient Assistance program?

Patients who are uninsured or have a coverage gap, with household income at or below 300% of the federal poverty level. The program ships Monoferric free directly to the provider, with no cap on the number of doses — no insurance billing involved. It covers the drug only; the office visit and the 96365 infusion fee are billed separately. Apply on monoferric-patient-solutions.com ↗

How much does the Monoferric Patient Solutions Copay Assistance Program Program cover in a year?

Up to $4,000 per calendar year, with each treatment capped at $2,000. The label is a single 1,000 mg infusion, repeated only if iron deficiency anemia recurs, so the $4,000 annual maximum covers two full treatments in the same year. Call 1-800-992-9022 for enrollment questions.

Does either Monoferric program cover the infusion fee or office visit?

No. Both the Co-pay Program and the Patient Assistance program cover the drug only. The 96365 infusion fee, $67.14 at the Medicare rate, and any office visit are billed separately, whichever program the patient uses.
