Commercial insurance: Eligible commercially insured patients may qualify for the Genentech Ophthalmology Co-pay Program. Eligible patients pay $0 toward the Lucentis drug charge, up to $15,000 a year, and the program adds up to $1,000 a year toward the 67028 injection administration copay; it does not cover supplies or physician fees beyond that. It excludes Medicaid, Medicare and other government insurance. View current program ↑
How the lines work: A Lucentis visit produces two charges: a drug line under J2778, 5 units per eye at the 0.5 mg wet AMD, RVO and mCNV dose or 3 units per eye at the 0.3 mg DME and DR dose, and a procedure line for the injection itself, 67028, reported per eye (RT or LT, or once with modifier 50 when both eyes are treated at the same visit). Each line carries its own patient share. The Genentech Ophthalmology Co-pay Program pays the drug line’s share in full, up to $15,000 a year, and separately contributes toward the 67028 charge.
Medicare: Manufacturer copay assistance is not available to patients using Medicare to pay for Lucentis; the Genentech Ophthalmology Co-pay Program explicitly excludes Medicare, Medicaid and other government plans. What exists instead is a Medigap policy that picks up the Part B coinsurance (see the cost page for the math), Medicaid dual eligibility where it applies, a foundation fund when one reopens, and the Genentech Patient Foundation for patients who are uninsured. The foundation funds: Nine funds are on file for Lucentis. The Assistance Fund's Macular Degeneration fund is on file; check each fund's current status on the funds list. The other eight are HealthWell Foundation's Macular Degeneration fund (up to $4,000 a year), Good Days' two retinal-disease funds, and five TotalAssist eye funds; check each fund's current status on the funds list. This page’s fund cards update automatically when one reopens.
Uninsured: The Genentech Patient Foundation provides Lucentis at no cost to qualifying uninsured patients, patients whose coverage falls short and some commercially insured patients who meet the program's rules with household income under $150,000. It ships the drug free to the practice; it does not cover the injection procedure or office-visit fees, so the practice still bills the patient for the 67028 charge, or writes it off. Why fund status matters: Manufacturer programs can change between: OPEN Accepting applications now · WAITLIST Not enrolling immediately · CLOSED Not accepting new applications. Every CareCost program card shows the status and the date it was verified.
Why the product name matters: Lucentis, Byooviz, Cimerli, Eylea, Eylea HD, Pavblu, Vabysmo and Beovu treat overlapping conditions but are separate products under separate HCPCS codes, each with its own manufacturer and, where one exists, its own assistance program. Confirm the product actually prescribed and billed before applying to a Genentech program.
Yes. The Genentech Ophthalmology Co-pay Program is Genentech’s current commercial-insurance program. Eligible patients pay $0 toward the Lucentis drug charge, up to $15,000 a year, and the program adds up to $1,000 a year toward the 67028 injection administration copay. Partially. The Genentech Ophthalmology Co-pay Program covers the drug charge (J2778) in full, up to $15,000 a year, and separately covers up to $1,000 a year toward the 67028 injection administration copay (RT or LT, or once with modifier 50 for both eyes). It does not cover a separate office-visit copay. View current program ↑
No. The Genentech Ophthalmology Co-pay Program, which pays a commercial patient’s Lucentis cost down to $0 up to $15,000 a year, excludes Medicare, Medicaid and other government insurance entirely. The Genentech Patient Foundation is a free-drug option for some insured patients too. It takes uninsured patients, patients whose plan does not cover Lucentis, and insured patients whose plan's out-of-pocket maximum is more than 7.5% of household income, with household income under $150,000 for up to 4 people. Nine funds are on file, including The Assistance Fund's Macular Degeneration fund and HealthWell's Macular Degeneration fund (up to $4,000 a year); check each fund's current status on the funds list. A Medicare patient’s real options are a Medigap policy that picks up the Part B coinsurance, or Medicaid dual eligibility where it applies. See Medicare cost math →

Apply at gene.com/patients/patient-foundation ↗ or call 888-941-3331; the Genentech Patient Foundation then provides Lucentis at no cost to qualifying uninsured patients, patients whose coverage falls short and some commercially insured patients who meet the program's rules with household income under $150,000. It ships the drug free to the practice but does not cover the injection procedure or office-visit fees. The practice still bills the patient, or writes off, the 67028 charge.
A claim the payer turned down leaves no approved amount for a copay program to reduce; the Genentech Ophthalmology Co-pay Program applies only to an approved claim, and it is not an appeals program. 19 of the 27 payers in CareCost’s ranibizumab coverage corpus require a documented trial of another product first, most often repackaged bevacizumab. Check the payer’s prerequisites before assuming the claim will be approved. Check coverage requirements →
Lucentis shares its class with Byooviz (Q5124, Biogen / Samsung Bioepis), Eylea (J0178, Regeneron), Eylea HD (J0177, Regeneron), Pavblu (Q5147, Amgen), Vabysmo (J2777, Genentech), Beovu (J0179, Novartis), billed under their own codes with their own manufacturer programs. The programs above apply to Lucentis only. If the payer requires Byooviz, Eylea, Eylea HD, Pavblu, Vabysmo or Beovu instead of Lucentis, use that maker’s program below.
Each Byooviz, Eylea, Eylea HD, Pavblu, Vabysmo or Beovu card shows the date we last checked that program. Copay cards also say whether they pay the infusion or injection fee. We have not checked a program for Pavblu yet.
