Commercial insurance: Eligible commercially insured patients may qualify for the Genentech Ophthalmology Co-pay Program. Eligible patients pay $0 toward the Lucentis drug charge, up to $15,000 a year, and the program adds up to $1,000 a year toward the 67028 injection administration copay; it does not cover supplies or physician fees beyond that. How the lines work: A Lucentis visit produces two charges: a drug line under J2778, 5 units per eye at the 0.5 mg wet AMD, RVO and mCNV dose or 3 units per eye at the 0.3 mg DME and DR dose, and a procedure line for the injection itself, 67028, reported per eye (RT or LT, or once with modifier 50 when both eyes are treated at the same visit). Each line carries its own patient share. The Genentech Ophthalmology Co-pay Program pays the drug line’s share in full, up to $15,000 a year, and separately contributes up to $1,000 a year toward the 67028 charge.
Medicare: Manufacturer copay assistance is not available to patients using Medicare to pay for Lucentis; the Genentech Ophthalmology Co-pay Program explicitly excludes Medicare, Medicaid and other government plans. What exists instead is a Medigap policy that picks up the Part B coinsurance (see the cost page for the math), Medicaid dual eligibility where it applies, a foundation fund when one reopens, and the Genentech Patient Foundation for patients who are uninsured. A claim the payer turned down leaves no approved amount for a copay program to reduce; the Genentech Ophthalmology Co-pay Program applies only to an approved claim, and it is not an appeals program. 19 of the 27 payers in CareCost’s ranibizumab coverage corpus require a documented trial of another product first, most often repackaged bevacizumab.
