Commercial insurance — LEQEMBI Copay Assistance brings the patient's cost on the drug charge to $0 per infusion, up to $10,000 per calendar year. The benefit applies to the Leqembi drug line only; it does not cover the 96365 administration line or other office-visit fees, and it excludes Medicare, Medicaid and other government insurance. Medicare — Medicare patients cannot use LEQEMBI Copay Assistance: federal law bars manufacturer copay assistance on Medicare, Medicaid and other government plans. A Medigap plan can pick up the Part B coinsurance instead — see the cost page for the Plan K and Plan L figures. A patient with both Medicare and Medicaid pays through Medicaid dual eligibility rather than manufacturer assistance. The LEQEMBI Companion Patient Assistance Program stays open to uninsured patients regardless of age. Coverage itself runs under NCD 200.3, coverage with evidence development, which requires registry enrollment; that is a coverage condition, not a source of payment assistance.

How the lines work — An 800 mg dose (10 mg/kg at 80 kg) draws two 500 mg vials, 1,000 mg total. The 800 mg administered is billed as the drug line, J0174 × 800 units; the 200 mg discarded from the second vial is billed separately on a JW line, J0174-JW × 200 units; the infusion itself is one line, 96365. Each line carries its own patient share under commercial coinsurance, and neither Eisai program pays the 96365 line or the coinsurance on the JW line.

Medicare patients can pair a Medigap plan or Medicaid dual eligibility to cover the remaining coinsurance; federal law bars manufacturer copay assistance itself.
