How Kisunla assistance works

Commercial insurance: The Kisunla Savings Card Program is Eli Lilly's commercial copay card. Eligible patients pay $0 for the drug, up to $10,600 a year. A Kisunla claim carries two lines, the drug (J0175 billed at 700 units for the 1,400 mg maintenance dose) and the 96365 infusion administration, and the card applies to the drug line only; the infusion charge is billed and collected separately. It excludes Medicare, Medicaid, and other government insurance. View current program ↑

Uninsured: Lilly Cares Foundation Patient Assistance ships Kisunla free to eligible uninsured patients and some Medicare patients who meet the program's rules on income-based eligibility. The shipment is the drug alone; the infusion charge still goes on the patient's bill.

Medicare: Manufacturer copay savings cannot be used when government insurance is paying for Kisunla; the federal Anti-Kickback Statute is why, per HHS OIG guidance on copayment coupons, and it is also why the Kisunla Savings Card Program excludes Medicare outright; Lilly Cares can help a Medicare Part B patient who has no supplemental coverage and an income. What is available instead is Medigap, which caps the Part B coinsurance under Plan K or Plan L, and Medicaid dual eligibility. On the 1,400 mg maintenance dose, a Medicare patient's 20% coinsurance is $599.33 on the $2,996.64 Medicare allows ($585.90 drug plus $13.43 infusion), and no manufacturer program reduces it. Coverage runs under NCD 200.3, coverage with evidence development: the prescriber enrolls the patient in a CMS-approved registry (ALZ-NET is the most used), a coverage condition, not an assistance program. No foundation fund is on file for either indication. See Medigap out-of-pocket by plan →

Why the diagnosis selector doesn't change the result: Neither Kisunla program is diagnosis-gated. Eligibility runs on the patient's insurance status, not on whether the diagnosis is mild cognitive impairment due to Alzheimer's disease (G31.84) or mild Alzheimer's disease dementia (G30.9); both qualify the same way for whichever program matches the patient's insurance.

Frequently asked questions

Is there a Kisunla copay card?

Yes. The Kisunla Savings Card Program is Eli Lilly's commercial copay card: eligible patients pay $0 for the drug, up to $10,600 a year. It covers the drug only and excludes Medicare, Medicaid, and other government insurance. Apply on kisunla.lilly.com ↗ Calculate cost with the program →

Does the Kisunla Savings Card cover the infusion fee?

No. The card applies to the drug line only, J0175 billed at 700 units at the 1,400 mg maintenance dose. The 96365 infusion charge is billed separately, and neither Lilly program pays it. View current program ↑

Can Medicare patients get Kisunla for free?

Not with the copay card: the Kisunla Savings Card Program excludes Medicare and Medicaid outright. Lilly Cares can help a Medicare Part B patient who has no supplemental coverage and an income at or below 500% of the federal poverty level. Other routes to lower cost are Medigap or Medicaid dual eligibility. Coverage runs under NCD 200.3, coverage with evidence development: the prescriber enrolls in a CMS-approved registry (ALZ-NET is the most used), a coverage condition, not an assistance program. See Medigap out-of-pocket by plan →

How do I apply for Lilly Cares?

Call 800-545-6962 or apply through kisunla.lilly.com. Lilly Cares ships Kisunla free to eligible uninsured patients and some Medicare patients who meet the program's rules on income-based eligibility; the infusion line still goes on the patient's bill. Apply on kisunla.lilly.com ↗

Is there a foundation fund for Kisunla?

No. No independent charitable foundation currently lists a fund for either Kisunla indication. Fund availability changes as foundations open and exhaust funding. Check today's status ↑

What if the plan denies Kisunla?

The Kisunla Savings Card Program reduces what a patient owes on an approved, paid claim; it does not apply to a denied claim. The denial has to be resolved through the payer's appeal or reconsideration process before either Lilly program has anything to apply to.

Kisunla shares its class with Leqembi (J0174, Eisai / Biogen), billed under its own code with its own manufacturer programs. The programs above apply to Kisunla only. If the payer requires Leqembi instead of Kisunla, use that maker’s program below.

Each Leqembi card shows the date we last checked that program. Copay cards also say whether they pay the infusion or injection fee.
