Commercial insurance: Eligible commercially insured patients may qualify for the EYLEA4U Copay Card. Eligible patients pay $5 per treatment toward the Eylea drug charge, up to $20,000 a year; the card does not cover the injection procedure, supplies or physician fees. It excludes Medicare, Medicaid and other government insurance. View current program ↑
How the lines work: An Eylea visit produces two charges: a drug line under J0178, 2 units per eye, and a procedure line for the injection itself, 67028, reported per eye (RT or LT, or once with modifier 50 when both eyes are treated at the same visit). Each line carries its own patient share. The EYLEA4U Copay Card pays the drug line’s share only, never the 67028 charge.
Medicare: Manufacturer copay assistance is not available to patients using Medicare to pay for Eylea; the EYLEA4U Copay Card explicitly excludes Medicare, Medicaid and other government plans. What exists instead is a Medigap policy that picks up the Part B coinsurance (see the cost page for the math), Medicaid dual eligibility where it applies, a foundation fund when one reopens, and Regeneron Patient Assistance for patients who are uninsured. Uninsured: Regeneron Patient Assistance provides Eylea at no cost to qualifying uninsured patients at or below 400% of the federal poverty level. It ships the drug free to the practice; it does not cover the injection procedure or office-visit fees, so the practice still bills the patient for the 67028 charge, or writes it off. A claim the payer turned down leaves no approved amount for a copay program to reduce; the EYLEA4U Copay Card applies only to an approved claim, and it is not an appeals program. 22 of the 28 payers in CareCost’s aflibercept coverage corpus require a documented trial of another product first, most often repackaged bevacizumab.
