Commercial insurance: Eligible commercially insured patients may qualify for the EYLEA4U Commercial Copay Card Program. Eligible patients may pay as little as $0 per treatment toward the Eylea drug charge, up to $20,000 a year; the card also pays up to $1,000 a year toward the injection procedure (not in MA or RI), and does not cover supplies or physician fees. How the lines work: An Eylea visit produces two charges: a drug line under J0178, 2 units per eye, and a procedure line for the injection itself, 67028, reported per eye (RT or LT, or once with modifier 50 when both eyes are treated at the same visit). Each line carries its own patient share. The EYLEA4U Commercial Copay Card Program pays the drug line’s share only, never the 67028 charge.
Medicare: Manufacturer copay assistance is not available to patients using Medicare to pay for Eylea; the EYLEA4U Commercial Copay Card Program explicitly excludes Medicare, Medicaid and other government plans.
