Benlysta shares its class with Saphnelo (J0491, AstraZeneca), billed under its own code with its own manufacturer programs. The programs above apply to Benlysta only. If the payer requires Saphnelo instead of Benlysta, use that maker’s program below.
Each Saphnelo card shows the date we last checked that program. Copay cards also say whether they pay the infusion or injection fee.

Commercial insurance — the BENLYSTA Copay Program brings eligible patients' drug cost to as little as $0 per dose, and unlike most manufacturer cards it also covers administration fees, up to $100 per administration, except for residents of Massachusetts and Rhode Island. The annual maximum is set at GSK's discretion; call 1-800-741-0375 for the current cap. The program excludes Medicare Part B/D, Medicaid, Medigap, VA, DoD, TRICARE, and state pharmaceutical assistance programs. View current program ↑
Medicare — the BENLYSTA Copay Program excludes Medicare, Medicaid and other government insurance, but the GSK Patient Assistance Program takes uninsured patients and Medicare patients who meet its income and other rules. Three funds could also help: HealthWell's Systemic Lupus Erythematosus, PAN's Systemic Lupus Erythematosus and HealthWell's AutoImmune Medicare Access; check each fund's current status on the funds list. A Medigap plan can pick up the Part B coinsurance instead; see the cost page for the Plan K and Plan L figures.
HealthWell's ANCA-associated vasculitis fund (up to $6,000 a year) is for a condition Benlysta is not FDA-approved for, so it does not apply here. The funds that match a Benlysta indication are HealthWell's Systemic Lupus Erythematosus fund ($15,000/year), the PAN Foundation's Systemic Lupus Erythematosus fund ($4,500/year), and HealthWell's AutoImmune Medicare Access fund ($2,800/year); check each fund's current status on the funds list.
How the lines work — 10 mg/kg for an 80 kg patient is 800 mg, billed as the drug line, J0490 × 80 units, drawn from two 400 mg vials with nothing discarded (JZ). The infusion itself is a separate line, 96365, for the 60-minute drip. Each line carries its own patient share under commercial coinsurance, and the BENLYSTA Copay Program's terms are among the few that reimburse part of that second line too; the example estimate below applies the card to the drug line only.
Yes. The BENLYSTA Copay Program brings eligible commercially insured patients' cost to as little as $0 per dose. The annual maximum is set at GSK's discretion — call 1-800-741-0375 for the current cap. Apply on GSK ↗ Calculate cost with the program →
Yes, partly. The program's terms list administration fees as covered along with the Benlysta drug charge, up to $100 per administration — one of the few manufacturer cards that pays any part of the 96365 line. Residents of Massachusetts and Rhode Island are not eligible for the administration reimbursement. View current program ↑
Yes, possibly. The BENLYSTA Copay Program excludes Medicare Part B and Part D, along with Medicaid, Medigap, VA, DoD, TRICARE and state pharmaceutical assistance programs. An uninsured patient's practice submits the enrollment; approved patients have Benlysta shipped directly to the provider, with no insurance billed for the drug charge. Income is assessed against GSK's maximum annual gross income tables by household size and region rather than a flat federal poverty level percentage; call 1-877-423-6597 for the current tables. A denial does not change the Benlysta assistance programs available; a commercially insured patient can still use the BENLYSTA Copay Program once coverage is resolved, and an eligible uninsured patient can apply to the GSK Patient Assistance Program regardless of the payer decision. Review the plan's prior authorization requirements on the coverage page.
