Commercial insurance — the BENLYSTA Copay Program brings eligible patients' drug cost to as little as $0 per dose, and unlike most manufacturer cards it also covers administration fees, up to $100 per administration, except for residents of Massachusetts and Rhode Island. The annual maximum is set at GSK's discretion; call 1-800-741-0375 for the current cap. Medicare — the BENLYSTA Copay Program excludes Medicare, Medicaid and other government insurance, and the GSK Patient Assistance Program is limited to uninsured patients, with Medicare Part D patients specifically excluded even if they fall into the coverage gap. The three funds that could otherwise help, HealthWell's Systemic Lupus Erythematosus, PAN's Systemic Lupus Erythematosus and HealthWell's AutoImmune Medicare Access, are all closed as of their last checks. How the lines work — 10 mg/kg for an 80 kg patient is 800 mg, billed as the drug line, J0490 × 80 units, drawn from two 400 mg vials with nothing discarded (JZ). The infusion itself is a separate line, 96365, for the 60-minute drip. Each line carries its own patient share under commercial coinsurance, and the BENLYSTA Copay Program's terms are among the few that reimburse part of that second line too; the example estimate below applies the card to the drug line only.
