Commercial insurance — the BENLYSTA Copay Program brings eligible patients' drug cost to as little as $0 per dose, and unlike most manufacturer cards it also covers administration fees, up to $100 per administration, except for residents of Massachusetts and Rhode Island. How the lines work — 10 mg/kg for an 80 kg patient is 800 mg, billed as the drug line, J0490 × 80 units, drawn from two 400 mg vials with nothing discarded (JZ). The infusion itself is a separate line, 96365, for the 60-minute drip. Each line carries its own patient share under commercial coinsurance, and the BENLYSTA Copay Program's terms are among the few that reimburse part of that second line too; the example estimate below applies the card to the drug line only.
