Commercial insurance — the Genentech Co-pay Assistance Program brings the patient's cost on the drug charge to $5 per infusion, up to $15,000 per calendar year. The benefit applies to the Actemra drug line only; it does not cover the 96365 administration line or other office-visit fees, and it excludes Medicare, Medicaid and other government insurance. Medicare — the Genentech Co-pay Assistance Program excludes Medicare, Medicaid and other government insurance, and the Genentech Patient Foundation's free-drug program is limited to patients who are uninsured or in a coverage gap, so an insured Medicare patient does not qualify for either. Two HealthWell Foundation funds that could otherwise help are both closed: AutoImmune Medicare Access, up to $2,800 a year, and Giant Cell Arteritis or Temporal Arteritis Medicare Access, up to $4,500 a year, last checked June 11, 2026. A Medigap plan can pick up the Part B coinsurance instead; see the cost page for the Plan K and Plan L figures.
One HealthWell Foundation fund is open today, ANCA-associated vasculitis, up to $6,000 a year, but Actemra is not FDA-approved for that condition, so it does not apply here. Every fund that does match an Actemra indication is closed: HealthWell's AutoImmune Medicare Access, HealthWell's Giant Cell Arteritis or Temporal Arteritis Medicare Access, and TotalAssist's Rheumatoid Arthritis fund.
How the lines work — an 8 mg/kg dose for an 80 kg patient with rheumatoid arthritis is 640 mg, billed as the drug line, J3262 × 640 units, drawn from one 400 mg vial and three 80 mg vials with nothing discarded. The infusion itself is a separate line, 96365, for the 60-minute drip. Each line carries its own patient share under commercial coinsurance, and neither Genentech program pays the 96365 line.

A denial does not change the Actemra assistance programs available; a commercially insured patient can still use Genentech Co-pay Assistance once coverage is resolved, and an eligible uninsured patient can apply to the Patient Foundation regardless of the payer decision.
